Pick up application at the Clerk of Court office or click here to down load the passport application. You need to follow the following instructions to complete the form.
Complete sections 1-20
Do not complete anything past 20
Bring the application in and sign in front of the Clerk of Court or Deputy.
This application must be accompanied by 2 (two) passport photos and a certified copy of your birth certificate.
